Practitioner Pricing Notes
*Some practitioners choose to offer additional services with the lab testing and prefer to charge for the lab testing and the additional services all together in the office. Marking up lab testing without providing additional services is prohibited. Practitioners must have a valid credit card on file with Physicians Lab when ordering practitioner-priced testing kits. Practitioner purchases resulting in a credit card decline may incur a $30 fee in addition to the cost of testing. Practitioner pricing is based on credit card payment at the time of order. Invoiced practitioner pricing may be higher.
