As a patient of Physicians Lab, you may register with our website to complete a personal history form and manage your laboratory testing orders.
We use the information you provide about yourself when generating medical results for your physician. The lab testing results along with the personal information provided by you is used as a tool for the ordering physician when analyzing the results of lab testing. Unless you request, in writing, to send this information to another qualified physician, we do not share this information with outside parties except to the extent necessary to complete testing, operations and payment under the HIPAA Privacy and Securities Act.
We use return email addresses to answer the email and requests we receive. Such addresses are not used for any other purpose and are not shared with outside parties.
Finally, we never use or share the personally identifiable information provided to us online in ways unrelated to the ones described above without also providing you an opportunity to opt-out or otherwise prohibit such unrelated uses.
